Accounts and clinical information are not the same thing.
Closing an identity account does not necessarily erase clinical records, third-party records, audit logs, backups or information retained under healthcare agreements and applicable law.

How to make a request.
1. Use your designated channel
Contact your ReportRocket or Glazco representative through the established support or contracting channel.
2. Identify the request
Specify whether it concerns your user account, eligible personal information, organization data or a clinical record. Do not send PHI through general inquiry channels.
3. Verify authority
Requests may require verification of identity, organization authorization, or the applicable covered entity’s instructions.
4. Receive a determination
The handling of a request depends on the information involved, the applicable agreements, technical capabilities and legal retention requirements.
03 / PATIENT INFORMATION
Patient records are managed through healthcare organizations.
If you are a patient seeking access to, amendment of or deletion of medical information, contact the hospital, radiology practice or other healthcare organization responsible for your care. That organization determines the appropriate response under applicable law, including HIPAA where applicable.
ReportRocket is a tool used by healthcare professionals; it is not a patient portal.
04 / IMPORTANT DISTINCTIONS
Retention and deletion limits.
Deletion may be limited by legal obligations, contractual requirements, legitimate security and fraud-prevention needs, dispute preservation, backup cycles and retention within third-party systems such as clinical reporting platforms. We do not promise universal deletion of every copy of information merely because an account is closed.
